Life After Hospital

Helping You Return Home Safely and Confidently

Leaving hospital can feel like a huge step.

Whether you are returning home yourself or supporting a parent, partner or loved one, it’s natural to have questions and concerns.

  • Will they manage the stairs?

  • Is the bathroom safe?

  • Do they need equipment?

  • Will they cope on their own?

  • Are carers needed?

  • Is the home still suitable?

An independent Occupational Therapy home assessment can provide reassurance and practical advice to help make the transition home as safe and successful as possible.

Returning Home After Hospital

Many people leave hospital before they have fully regained their strength or confidence.

Simple everyday activities may suddenly become much more difficult.

Our assessment considers how someone is managing with:

  • Walking around the home

  • Getting in and out of bed

  • Standing from chairs

  • Using the toilet safely

  • Bathing and showering

  • Managing the stairs

  • Preparing meals

  • Everyday activities around the home

We identify any risks and provide practical recommendations to improve safety and independence.

Is Home the Right Place?

Sometimes families simply want reassurance.

Following a hospital admission, it’s common to wonder whether someone is ready to manage safely at home.

An independent assessment can help identify:

  • Current abilities

  • Areas of concern

  • Falls risks

  • Equipment that may help

  • Home adaptations that should be considered

  • Support that may be needed

Our aim is always to help people remain as independent as possible while reducing unnecessary risks.

Equipment and Home Adaptations

Following illness or injury, simple changes can make a significant difference.

Recommendations may include:

  • Grab rails

  • Toilet equipment

  • Shower seating

  • Bed equipment

  • Walking aids

  • Stairlifts

  • Level access showers

  • Minor home adaptations

Recommendations are always based on individual need and are completely independent.

OTLife does not sell equipment or receive commission from suppliers.

Supporting Families

Hospital discharge can be stressful for families.

You may suddenly find yourself helping with shopping, meals, medication or personal care while trying to decide what support is needed.

Our assessment provides clear professional guidance, helping families understand what practical changes may improve safety and confidence at home.
